The successful candidate:
Must be able to work Monday-Friday, 9:30a-6:00p.
Review, verify, and dispense safe, evidence-based medication therapies in accordance with established policies, procedures, and clinical guidelines
Assess medication orders for clinical appropriateness based on labs, vitals, symptoms, disease states, comorbidities, and medication history
Optimize medication therapy and document all pharmacist interventions in the medical record
Provide medication education and discharge counseling to patients and caregivers, including new start oral chemotherapy and symptom management
Collaborate with interdisciplinary teams, including inpatient/outpatient providers and medical oncology teams, to support patient treatment plans
Participate in interdisciplinary rounds and serve as a medication therapy resource for providers and clinical staff
Communicate effectively with providers and act as a liaison to resolve medication order and drug-related issues
Prepare and dispense sterile compounded products in compliance with USP <797> and USP <800>, and supervise supporting technician staff
Direct, supervise, and support pharmacy technicians in daily operations
Participate in departmental and hospital quality assurance, patient safety, and regulatory compliance initiatives
Assist in education and assessment of pharmacy staff, providers, clinical staff, patients, caregivers, and pharmacy learners
Perform additional duties as assigned
Qualifications
Your qualifications should include:
• Graduate of an ACPE-accredited College of Pharmacy
• Active license with the State Board of Pharmacy
• Doctor of Pharmacy or other advanced degree highly preferred
• Minimum of 2 year’s institutional pharmacy experience or strong interest in institutional and oncology pharmacy
• Familiar with using pharmacy computer systems and other technology, including BD Pyxis Medstation
