We are seeking a compassionate and experienced Senior Nurse Navigator to join our hematology/oncology team in the ambulatory clinic setting. In this rewarding role, you'll be a key advocate and resource for patients facing complex cancer diagnoses— offering personalized education and support, especially around chemotherapy treatment and ensuring a seamless, timely care experience.
You’ll work closely with physicians, research teams, and transplant coordinators to coordinate diagnostic processes and care plans, advocating for each patient every step of the way. As a trusted liaison between specialties, you’ll help ensure that no detail is missed and that follow-up care is prompt and effective.
As a successful candidate, you will:
Serve as the primary point of contact for new patients, guiding them through initial consultations, diagnostic testing, and treatment planning.
Provide individualized education on chemotherapy and other treatment protocols.
Collaborate with physicians, research and transplant coordinators, and interdisciplinary teams to ensure timely diagnoses and coordinated care.
Act as a consistent communicator between specialties, ensuring continuity, timely follow-ups, and patient-centered support throughout the care journey.
Help patients and their families feel informed, supported, and empowered at every stage.
Your qualifications should include:
Bachelor’s degree
Five years of professional nursing experience to include three (3) years of ambulatory oncology experience.
Registered Nurse in the State of California
One of the following certifications required within one (1) year of hire: OCN, AOCN, ONN-CG, ONN-CGSM, AOCNP
Preferred education, experience, and skills:
With preferred degree, two (2) years of experience in an ambulatory oncology setting.
