Pay Rate: $61.65 - $89.4 per hour

Clinical Nurse III – Diabetes Educator

We are seeking a dedicated and experienced Diabetes Educator Nurse to join our endocrinology service team. The successful candidate will play a crucial role in providing comprehensive diabetes education and facilitating care coordination for patients with diabetes.  The position involves direct patient care, educational program development, and collaboration with the endocrinology team to ensure high-quality patient outcomes in both the clinic and inpatient hospital setting. This opportunity is for full-time, 10-hour shifts, and 40 hours per week. The clinic is open Monday - Friday. 

Up to $20,000 bonus is available!

As a successful candidate, you will: 

  • Patient Education: Provide individualized diabetes education to patients and their families, including instruction on blood glucose monitoring, medication management, insulin administration, diet, and lifestyle modifications.
  • Care Coordination: Collaborate with endocrinologists, primary care providers, dietitians, and other healthcare professionals to develop and implement comprehensive care plans for patients with diabetes.
  • Clinical Support: Conduct patient assessments, review medical histories, and monitor patient progress. Adjust care plans as needed based on patient feedback and clinical outcomes.
  • Program Development: Develop, implement, and evaluate diabetes education programs and materials. Stay updated with the latest diabetes management practices and integrate them into educational programs.
  • Documentation: Maintain accurate and up-to-date patient records, including education provided, patient progress, and care plans. Ensure compliance with clinic policies and regulatory requirements. 
  • Perform pre-procedure phone calls for appointments.
  • Triage patient concerns via telephone and inbox messages using established protocols by documenting all encounters with EMR.
  • Promote wellness and prevention strategies to oncology patients, ensuring they are equipped with knowledge about self-care and survivorship.

 Your qualifications should include:

  • Bachelor’s degree in nursing or master’s degree in nursing
  • Three to five years related experience. 
  • Current California Registered Nurse license
  • Current BLS certification from American Heart Association
  • CDCES is needed. 

Preferred:

  • Master’s degree in nursing or related field
  • National certification (must obtain within three years and must be maintained thereafter to retain Clin III status)
